Treatment recommendations and visit frequency are based on your individual condition and clinical findings.

Patients returning after an extended lapse in care may require an updated evaluation before resuming treatment.

Cancellations

Please provide at least 24 hours’ notice if you need to cancel or reschedule.

Missed appointments or late cancellations may result in a fee.

Arrival

Please arrive on time and wait in the lobby until your appointment begins.

Payment

Payment is due at the time of service. Patients may also prepay online when scheduling.

Insurance

Please text or email a copy of your insurance card and date of birth prior to your appointment if using insurance benefits.

Insurance verification is provided as a courtesy and is not a guarantee of payment. Patients are responsible for any non-covered charges.

Integrity Chiropractic maintains a professional doctor-patient relationship at all times Care decisions, scheduling, and financial matters are discussed directly with the patient unless the patient is a minor.

Appropriateness of Care

If another provider is better suited for your condition, an appropriate referral will be recommended.
